We offer services ranging from comprehensive short-term rehabilitation and transitional care to Alzheimer's and dementia care. Day to day as a Licensed Practical Nurse - LPN As a new Full-Time Licensed Practical Nurse (LPN) at Hilltop Manor, you can expect a dynamic and hands-on role. Your daily responsibilities will include providing direct patient care, monitoring vital signs, and assisting with daily activities to promote wellness. You will collaborate closely with registered nurses and other healthcare professionals to develop and implement patient care plans. On a typical shift from 11 PM to 7 AM, you will be at the forefront of nighttime patient care, addressing immediate needs and ensuring a safe, comfortable environment. Each night, you will conduct routine assessments and maintain accurate patient records. You will also be responsible for administering medications and treatments as prescribed. Working every other weekend, you will participate in team meetings and contribute to a culture of continuous improvement, reinforcing our commitment to excellence in patient-centered care.
