About The Position

"Navitas Healthcare, LLC" is seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) for a 16-week contract assignment in a Long-Term Care / Skilled Nursing Facility setting. The LPN will provide direct patient care under the supervision of a Registered Nurse (RN) or physician, ensuring high-quality care and patient safety in a fast-paced environment.

Requirements

Active Licensed Practical Nurse (LPN) license in the state of New York
Basic Life Support (BLS) certification required (AHA preferred)
Minimum 1 year of recent LPN experience in a hospital or long-term care setting preferred
Strong knowledge of basic nursing practices and clinical procedures
Ability to work effectively under supervision in a team-based environment
Proficiency in EMR systems and standard documentation practices
Excellent communication, time-management, and organizational skills

Responsibilities

Perform focused patient assessments and report any changes in condition to the RN or healthcare provider
Administer medications, treatments, wound care, and IV therapies in accordance with scope of practice and facility guidelines
Assist in the development and implementation of individualized patient care plans
Collaborate with RNs, physicians, and interdisciplinary teams to support patient care and discharge planning
Accurately document patient care activities, observations, and responses in the EMR system
Provide basic patient and family education as directed by the RN
Respond promptly to patient needs and communicate effectively with the healthcare team
Maintain compliance with infection control protocols, medication administration standards, and safety regulations
Assist with patient admissions, transfers, and discharges
Support unit operations and contribute to a positive team environment
