About The Position

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care in a Long-Term Care (LTC) setting. The LPN will work under the supervision of a Registered Nurse or physician, supporting patient care, monitoring health status, and ensuring a safe and comfortable environment for residents.

Requirements

• Active Colorado (CO) or Compact State LPN license required
• Minimum 2 years of experience in Long-Term Care or relevant specialty required
• Current BLS certification (AHA) required
• Strong clinical, communication, and organizational skills
• Ability to work in a fast-paced and team-oriented environment

Responsibilities

• Provide direct patient care in a long-term care environment
• Monitor and record vital signs including blood pressure, temperature, pulse, and respiration
• Administer medications and injections as prescribed
• Perform wound care, dressing changes, and specimen collection
• Assist with patient assessments and report changes in condition to the RN or physician
• Support daily patient care needs and ensure comfort and safety
• Prepare patients for procedures and assist as needed
• Maintain accurate and timely documentation
• Follow infection control, safety, and facility protocols
