About The Position

We are seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our Rochester-area home care team on a per diem basis. This position offers independence, flexibility, and meaningful one-on-one care for patients who prefer to remain in the comfort of their homes. Requirements

• Current and active LPN license in the State of New Hampshire (required).
• Valid driver’s license and reliable transportation.
• Prior experience in home care, long-term care, or rehabilitation preferred.
• Strong communication and documentation skills.

Responsibilities

• Conduct in-home patient assessments, medication management, and wound care.
• Monitor and document changes in patient condition and report to supervising RNs or physicians.
• Educate patients and caregivers on treatment plans, medications, and home safety practices.
• Maintain accurate, timely documentation using electronic charting.
• Collaborate with multidisciplinary care teams to ensure coordinated and effective patient care.
