About The Position

"Navitas Healthcare, LLC" is sseking for an experienced Licensed Practical Nurse (LPN) to provide comprehensive patient care in a Long Term Care setting. The role includes medication administration, patient monitoring, and supporting residents with complex medical needs.

Requirements

• Active LPN license (required upon acceptance)
• Minimum 2 years of Long Term Care (LTC) experience
• Current BLS/BCLS certification
• Flu vaccination or approved exemption required

Nice To Haves

• Experience with Meditech EMR

Responsibilities

• Perform patient assessments and provide prescribed treatments
• Administer medications and monitor patient responses
• Report significant changes in patient condition to MD/NP
• Assist with admissions and discharges
• Support activities of daily living (ADLs)
• Provide specialized care, including: Gastrostomy tube care, Catheter care, CPAP support, Nebulizer treatments, Wound vac management
• Maintain accurate documentation in Meditech EMR
• Ensure compliance with care standards and safety protocols
