About The Position

"Navitas Healthcare, LLC" is an experienced Licensed Practical Nurse (LPN) to provide high-quality care in a Long Term Care setting. The role requires strong clinical judgment, independence, and prior experience in skilled nursing or LTC environments.

Requirements

• Active, unrestricted Colorado LPN License
• Minimum 3 years of Long Term Care (LTC) experience
• Current BLS/BCLS certification
• Strong clinical judgment and patient care skills
• Ability to work independently in a long-term care environment
• Strong documentation and communication skills
• Team collaboration and resident-focused care approach

Nice To Haves

• Experience with MDS documentation
• Experience using PointClickCare EMR

Responsibilities

• Provide direct patient care including bathing, feeding, and toileting
• Administer medications and treatments as prescribed
• Monitor patient conditions and report changes promptly
• Maintain accurate and timely documentation
• Assist with care planning and MDS documentation (preferred)
• Ensure compliance with safety and infection control standards
• Collaborate with interdisciplinary healthcare teams
