Your Role and Impact: Lead with Expertise: Upon referral, conduct comprehensive evaluations of patients’ physical function—including strength, mobility, motor control, joint health, pain levels, balance, posture, and cardiopulmonary performance. You’ll assess vitals, endurance, gait, and daily living capabilities with precision and compassion. Create Impactful Care Plans: Design personalized, patient-centered treatment plans that promote optimal movement, safety, and long-term wellness. Your clinical insight will guide each step of the recovery journey. Deliver Innovative Therapy: Implement and delegate care using a wide range of therapeutic interventions—from heat, cold, and light therapy to mobilization and targeted exercise. You’ll use your clinical judgment to tailor treatments that truly work. Collaborate for Continuity: Work closely with a multidisciplinary team—including Rehab Therapists, Nurses, and Social Workers—to ensure seamless care coordination. You’ll also play a key role in discharge planning and connecting patients with community resources to support their ongoing recovery. Knowledge, Skills and Abilities: Must be flexible, a team player, and able to work efficiently in a fast-paced environment. Demonstrates strong time management skills and the ability to prioritize daily assignments effectively. Able to read, understand, and follow oral and written instructions, and communicate information clearly. Proficient in basic computer skills, including email and internet use. Strong working knowledge of Windows-based programs, including word processing, spreadsheets, databases, and presentation software. Working knowledge of State Practice Act guidelines and Federal Medicaid and Medicare regulations. Problem Solving and Decision Making Exercises sound judgment and discretion while performing duties independently and consistently with minimal supervision. Demonstrates strong analytical skills to gather and interpret information from multiple sources. Responds appropriately to changing circumstances and resolves issues effectively. Maintains strict confidentiality at all times. Bachelor’s Degree in physical therapy required, Master’s or DPT preferred. CT PT license and BLS certification required. Open to recent graduates. One year experience in acute care or outpatient setting preferred. Position may support the Waterford office if needed in the future. We take great care of careers.
