This position is part of the Mental Health Crisis Care Team, which provides care both in an outpatient clinic and across various Emergency Department settings via Telehealth with one in person shift. The clinician will engage in crisis assessments, therapeutic interventions, Diagnostic Assessments to access Programmatic care, while also developing and directing patient care and disposition plans. Building strong relationships with psychiatry, medical providers, and interdisciplinary teams is essential. Since 2002, the team has been dedicated to supporting youth and adults during crises, improving community health through innovation and partnerships. This position will be Tuesday – Friday with every other Saturday day and evening shifts. This is a 0.9FTE position and consists of 6 shifts at transition clinic at the outpatient level and 3 ED shifts per pay period. One in person shift with the transition clinic will be required at our St. Paul wellness hub location. The clinician may Conduct Crisis and Diagnostic assessments, Determine clinical interventions, Facilitate therapy, and Provide education for patients and their families. The role involves managing treatment planning, developing transition and discharge plans, and functioning as an interdisciplinary team member.
