Requirements
- Licensed Social Worker (LMSW or LCSW) in DC, Maryland, or Virginia (or eligible)
- Minimum of 1 year of clinical or medical social work experience required
- Strong communication, empathy, and problem-solving skills
- Knowledge of community resources and care coordination
- Ability to work independently and manage a flexible schedule
Nice To Haves
- Home health or healthcare experience preferred
Responsibilities
- Conduct social work assessments in patients’ homes
- Identify social, emotional, and environmental needs
- Provide counseling and emotional support to patients and families
- Assist with community resources, discharge planning, and care coordination
- Support patients with housing, financial, and support services
- Collaborate with nurses, therapists, and physicians
- Complete accurate and timely EMR documentation
