Requirements
- Master’s Degree in Social Work
- Five plus years clinical social work experience
- Valid and current Licensed Clinical Social Worker or Licensed Medical Social Worker license
- Ability to make psycho-social assessments and develop and implement viable care plans
Responsibilities
- Guides and counsels individuals and families in crisis, ranging from the unemployed to children who need foster homes to elderly people who have no one to care for them
- Assists clients in obtaining government funds, education, or treatment
- Secures information, such as medical, psychological, and social factors contributing to client's situation, and evaluates these and client's capacities
- Asks clients to explain problems, such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine nature and degree of problem
- Helps patient and family through individual or group conferences regarding plans for meeting needs, and aids client to mobilize inner capacities and environmental resources to improve social functioning
- Helps client to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and client's part in creating them
- Develops service plan designed to restore patient to optimum social and health adjustment within patient's capacity
- Refers clients to community resources and other organizations, and liaises with other social service agencies.
- Utilizes community resources to assist patient to resume life in community or to learn to live within limits of disability
- Prepares patient histories, service plans, and reports.
- Reviews service plan and performs follow-up to determine quantity and quality of service provided client and status of client's case
- Maintains client records in compliance with organizational goals and objectives, as well as external regulatory guidelines
- Other duties as assigned
