About The Position

The Licensed Therapist (LCSW) is responsible for superb assessment, evaluation, diagnostic, and synthesis skills. Essential to the role is client engagement and the ability to establish a therapeutic relationship. The therapist must be capable of providing timely and accurate comprehensive clinical assessments (CCA), demonstrating knowledge of NC clinical coverage policy definitions, level of care tools (i.e., LOCUS and ASAM), and a commitment to immediately engaging clients in the appropriate level of care. This skilled professional drives the overall direction of care, facilitating an excellent person-centered planning process. The therapist acts as a strong advocate for the client, ensuring timely and proper care based on diagnostic assessment results, and represents the purpose, vision, and mission of Southeastern Integrated Care, Inc.

Requirements

  • Master’s degree in clinical or counseling Psychology, Social Work, or a related subject area, or a bachelor’s degree with requisite experience in a similar field that meets QP definition.
  • Possess a NC state clinical license or provisional license (i.e., PHD, LCSW, LPC, LCMHC, LCAS).
  • Excellent engagement skills.
  • Excellent communications skills.
  • Excellent synthesis of information from the client, former records, and collateral skills.
  • Ability to work within EMR and familiarity with adjunct diagnostic assessment tools.

  • Excellent working knowledge of NC clinical coverage policies for behavioral health.
  • Excellent documentation skills.
  • Ability to work in a fast-paced environment both independently and collaboratively with team members.
  • CPR, first aid, NCI, and/or role-related certification (or obtain within 3 months of hire).
  • Previous work providing in-home or community-based therapy services.

Responsibilities

  • Conducting Comprehensive Clinical Assessments (CCA)/ Diagnostic Assessments and Person-Centered Planning in accordance with NC Clinical Coverage Policy No:8A-5, DLA-20 Guidelines and NC APSM 45-2 Records Management and Documentation Manual.
  • Ensuring CCA/ Diagnostic Assessments include all required elements: description of presenting problems, chronological general health, past trauma history and behavioral health history, current medications, review of biological, psychological, familial, social, developmental and environmental dimensions, evidence of beneficiary and legally responsible person’s participation, analysis and interpretation of assessment information with appropriate case formulation, diagnosis using DSM-5, recommendations for additional assessments, services, supports or treatment, signature and date by licensed professionals, and evidence of an interdisciplinary team service note.
  • Maintaining accurate, timely, objective, specific, concise, descriptive, consistent, comprehensive, logical, reflective of thought processes, clear, and person-centered documentation.
  • Collaboration and partnership with community agencies.
  • Provide services based on the principles of wellness self-management, personal recovery, natural supports, coping skills, self-advocacy skills, and development of independent living skills.
  • Assist clients with self-determination and decision-making.
  • Model recovery values, attitudes, beliefs, and personal action to encourage wellness.
  • Teach and promote self-advocacy and empower clients to use their legal rights.
  • Respect and protect client confidence and patient confidentiality by keeping all sensitive and client information private.
  • Other duties as assigned.
