About The Position

The Lead Social Worker is responsible for assessing, counseling, intervening, implementing and educating the patient, family/significant others to facilitate movement throughout the continuum of care.

Requirements

• Bachelor’s degree
• 2 –4 years related Experience
• NJ Licensed Clinical Social Worker
• Ability to communicate clearly in English, both orally and in writing
• Knowledge of the discharge planning process
• Knowledge of Medicare/Medicaid Regulations; community resources
• Knowledge of JCAHO guidelines (continuation of care)
