CentraState Medical Center has an employment opportunity available for a Social Worker (LSW/MSW). This position provides discharge planning assistance, supportive counseling, and advocacy services to assigned patients and their families.

Requirements

  • Master's degree in Social Work or in the related fields of Guidance and Counseling, Human Services, Marriage and Family Counseling, Psychology, Sociology, and Vocational/Disability Rehabilitation, from an accredited college or university required.
  • Internship required.
  • NJ Licensed Social Worker or Licensed Certified Social Worker required.
  • Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team.

Nice To Haves

  • Previous experience in medical social work or post-acute facility preferred.

Responsibilities

  • Assesses patient's social services and discharge planning needs and their ability to provide for those needs post discharge.
  • Identifies situations indicating possible abuse, dependencies, growth and development problems and follows up appropriately.
  • Provides support to patients and families coping with illness and/or hospitalization.
  • Follows regulations while working with patients and/or families for post-acute placement.
  • Makes recommendations regarding needs of patients and suggestions for providing more effective comprehensive care and social services.
  • Refers patients to community or other appropriate services post discharge.
  • Reports possible abuse cases to appropriate community agencies for further investigation.
  • Documents all assessments, referrals, and interventions in the patient’s medical record.
  • Maintains professional relationships with post-acute facilities for referrals and placements.
  • Attends multi-disciplinary rounds to support length of stay goals.
  • Assesses patients when requested by nursing, patient/family and/or physician.
  • Works with nursing, ancillary and medical staff with high degree of professionalism and tact.
  • Attends and/or facilitates family meetings when requested.
