Social Worker, LSW
About The Position
Social Worker will coordinate and provide social services for patients of all ages in the patient's care setting. Social workers play an essential role in many of the aspects of patient care, including helping patients and their families navigate the medical system, assessing and monitoring patients' and family members' mental and emotional health, providing short term counseling and therapy, and communicating patient needs and concerns to the larger medical team. Requirements
- Master’s Degree in social work is required.
- Bachelor's degree in social work plus five years of social work or social service clinical experience may be considered in lieu of a master's degree.
- Licensed as a Certified Social Worker (CSW) in the state of Wisconsin or Licensed Social Worker (LSW) in the state of Illinois is required.
- Basic Life Support (BLS) is required.
- In roles which travel is required, a valid driver's license, automobile insurance and own transportation is required.
Nice To Haves
- Prior experience in social work or past social service clinical experience directly transferrable to a healthcare environment is preferred.
- 2 years of home care, hospice, discharge planning or related experience is preferred.
- Wisconsin: Licensed as a Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), or Licensed Clinical Social Worker (LCSW) is preferred.
- Illinois: Licensed as a Licensed Clinical Social Worker (LCSW) is preferred.
