About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency operating throughout Arizona, offering residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals facing crisis, opioid use disorder, homelessness, and mental illness. CBI is a non-profit provider of substance use and behavioral health programs, utilizing cutting-edge, evidence-based models. A Lead Navigator is an individual in recovery from AOD use and/or GMH for at least one year, with a minimum of 90 days of experience as a Navigator or Peer Support Specialist. The primary responsibility of this role is to work with individuals experiencing homelessness, providing outreach and supportive services to help them secure housing. The Lead Navigator acts as a liaison, coordinating and monitoring community-based services (both RBHA and Non-RBHA) and facilitating their delivery by supporting the individual. This role involves actively seeking out individuals with the highest acuity and vulnerabilities, conducting outreach, engagement, case management, and providing ongoing wrap-around supportive services. Additionally, the Lead Navigator mentors Navigator IIs, assisting them with onboarding, program understanding, and fulfilling program requirements. The Lead Navigator is proficient in coordinated entry, housing referral, leasing up, wrap-around services, and discharge processes. This position is part of a multidisciplinary team, collaborating with Patient Care Techs, Registered Nurses, Counselors, and Clinical Leads, and coordinates care with external community providers and stakeholders.

Requirements

  • In recovery from AOD use and/or GMH for an appropriate period (greater than or equal to 1 year)
  • At least 90 days experience as a Navigator or Peer Support Specialist
  • Highschool diploma or GED
  • Current AZ Driver's License
  • Clear 39-month Motor Vehicle Record
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
  • Arizona Fingerprint Clearance card, site specific

Nice To Haves

  • Associates Degree (or higher) in a field related to behavioral health
  • 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations

Responsibilities

  • Working with individuals who are homeless, providing outreach and supportive services to assist the member with ending their homelessness
  • Coordinating and monitoring RBHA and Non-RBHA community-based services

  • Conducting outreach, engagement, case management, and ongoing wrap around supportive services to the individual
  • Mentoring Navigator IIs to help them onboard, understand the program, and fulfill program requirements
