CBI is recognized as a premiere non-profit provider of fully integrated behavioral and mental health programs, emphasizing prevention, education, and treatment through cutting-edge, evidence-based models.A Lead Navigator is an individual who has been in recovery from AOD use and/or GMH for at least one year and possesses a minimum of 90 days of experience as a Navigator or Peer Support Specialist. The primary function of this role is to engage with individuals experiencing homelessness, providing outreach and supportive services to facilitate an end to their homelessness. The Lead Navigator acts as a crucial link for patients, coordinating and monitoring both RBHA and Non-RBHA community-based services and ensuring their effective delivery. This position involves actively seeking out individuals with the highest acuity and greatest vulnerabilities, and is responsible for conducting outreach, engagement, case management, and ongoing wrap-around supportive services. Furthermore, the Lead Navigator serves as a mentor to Navigator II staff, assisting with onboarding, program comprehension, and fulfillment of program requirements. Lead Navigators are expected to have mastered processes such as coordinated entry, housing referral, leasing up, wrap-around services, and discharge. They collaborate within a multi-disciplinary team, including Patient Care Techs, Registered Nurses, Counselors, and Clinical Leads, and are responsible for coordinating care with external community providers and stakeholders.
