The Lead Case Manager plays a pivotal role in coordinating and overseeing the provision of comprehensive case management services to children in care and their families/sponsors. The Lead Case Manager will lead a team of case managers, case aides and in some instances an Assistant Lead Case Manager. This position will provide guidance, support, and supervision to ensure the delivery of high-quality case management services. The Lead Case Manager’s responsibilities will include assessing children needs, developing individualized service plans, coordinating resources, child advocacy, and monitoring progress towards reunification or program discharge. Additionally, the Lead Case Manager will collaborate with internal and external stakeholders to facilitate access to necessary services and promote positive outcomes for children in care.
