About The Position

The Lead Case Aide is responsible for the delivery of youth skills-building programming. He/she is responsible for the submission of service plans and other assessments required for the compiling of a comprehensive case file found on Southwest Key Programs designated web-based system with supporting documentation maintained in physical file. The Lead Case Aide maintains a flexible, organized and efficient work schedule and is subject to work more than a normal work day and be on call to facilitate program services.

Requirements

  • Minimum GED or High School Diploma with some college hours or 1-2 years working with at risk youth.
  • Cleared criminal background check from appropriate entity.
  • Must maintain a valid state driver’s license.
  • Must be able to obtain First Aid and CPR Certification.
  • Must be able to perform physical holds.
  • Be able to drive, sit, stand and walk for extended periods of time.
  • Daily use of various office equipment.
  • Bending, stooping, climbing up stairs and lifting up to 15 lbs. required to complete daily tasks.

Nice To Haves

  • Individuals with a Bachelors degree in Human Services field preferred.

Responsibilities

  • Deliver and coordinate skills-building programming, homework and tutorial assistance, and recreational activities for youth.
  • May be regularly assigned a small case load and required to serve as Interim case manager.
  • Provide direct supervision for youth at all times.
  • Direct case aides
  • Promote and encourage positive interaction among youth participants.
  • Facilitate Youth intake and assessment interviews.
  • Develop goal-oriented service plans.
  • Ensure all daily documentation is completed and entered into SWK database in a timely and efficient manner.
  • Provide advocacy as specified by Southwest Key Programs and contract guidelines.
  • Manage, organize and implement an efficient file system to include: documents, records, and reports.
  • Facilitate individual and group counseling.
  • Provide crisis intervention for students and families.
  • Serve as a positive role model for all students
  • Provide student supervision and structure in a safe and therapeutic environment.
  • Assist students in developing age appropriate socialization skills.
  • Accommodate students with special needs.
  • Perform conflict resolution and have excellent communication skills.
  • Provide youth transportation to and from center in the company van.
  • Able to react to change productively and handle other essential tasks as assigned.
