About The Position

The Children’s Lead Coordinator plays a pivotal role in implementing and supporting family-centered services across Illumination Health + Home family shelters. This position oversees Care-Coordinators at multiple shelter locations, ensuring consistency and quality of service delivery. The Children’s Lead is responsible for coordinating and monitoring family shelter intakes, ensuring that all intake processes are completed in a timely manner and documented according to established program procedures. In this role, the Lead provides critical communication, scheduling, and logistical support to sustain and strengthen community partnerships. They also support the Associate Director of Family Programs in managing daily shelter operations and assisting in conducting audits to ensure compliance with contractual requirements and organizational standards.

Requirements

  • 2 years’ relevant experience.
  • Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
  • Valid CA Driver’s License and eligibility for company vehicle insurance.

Responsibilities

  • Complete audits as required to ensure program compliance with contracts and funding requirements.
  • Request shelter referrals through HMIS and the family-coordinated entry system.
  • Screen shelter referrals and schedule intakes with families.
  • Complete intake paperwork with families and obtain vital documents during intakes.
  • Screen all incoming households for health insurance coverage and guide staff to assist uninsured clients in obtaining enrollment.
  • Create and maintain both physical and electronic case files for all new shelter participants.
  • Collect vital documentation at intake and track outstanding documents for follow-up.
  • Collaborate with agency and community partners to schedule on-site programming and educational classes for shelter families.
  • Maintain accurate and up-to-date case notes, documentation, and client records in both KIPU and HMIS systems.
  • Track program services, outputs, and outcomes for data collection, reporting, and evaluation.
  • Ensure Enhanced Case Management (ECM) services are consistently offered and accessible to all clients across family shelters.
  • Provide support to Family Navigators, ensuring service delivery is trauma-informed and client-centered.
  • Train and guide Family Shelter staff in program procedures, best practices, and client engagement.
  • Meet regularly with Case Managers to review ECM documentation and ensure case notes are current and complete.
  • Report any day-to-day or shelter operations to management and respond promptly to on-site situations and client needs.
  • Delegate essential tasks (e.g., intakes, referrals, screenings) to Family Navigators as needed.
  • Coordinate staff participation in events, health fairs, and clinics hosted by IF or partner agencies.
  • Lead regular case conferencing sessions with program staff to review client progress and care plans.
  • Monitor case files and documentation to ensure compliance with program standards and contract requirements.
  • Ensure staff prepare rooms and family spaces prior to new client move-ins.
  • Collaborate with Illumination Health + Home departments such as Communications and Volunteer Services to enhance the shelter experience.
  • Complete referrals and service linkages for children and families as appropriate.
  • Ensure compliance with all contractual obligations through regular monitoring of documentation and data records.
  • Led team efforts in achieving contractual goals and desired program outcomes.
  • Model and coached staff in utilizing a trauma-informed approach in client interactions and service delivery.
  • Demonstrate a strong commitment to Illumination Foundation’s mission and core values by fostering respectful and harmonious relationships across all levels of the organization.
  • Motivate team members while quickly adapting to new skills and procedures to enhance team effectiveness.
