Targeted case management program that provides community support for adults who have been diagnosed with a serious mental illness, some of whom have been diagnosed with a co-occurring substance use disorder. Keystone is a multi-disciplinary case management program, with prescribing and nursing embedded in the team, serving people that, due to mental health and/or substance use diagnosis, could benefit from a case manager and psychiatry services. We serve adults in Dane County, and the program is voluntary. Services are recovery-oriented, trauma informed, person-centers and strengths-based. Case Managers (and interns) do the majority of their work in the community, addressing the various needs of housing, accessing medical or substance use services, medication management, finding educational or vocational resources, assisting in navigating the criminal legal system, or providing supportive counseling. Interns will have the opportunity to participate in team meetings and clinical consultations, shadow case managers, and develop their own professional relationships with consumers, providing direct services, case management activities, advocacy and support. Interns will need to have a valid driver’s license and a vehicle. Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.
