About The Position

Forward Solutions is a Community Support Program (CSP) for people who have a serious mental illness and need interdisciplinary services to maintain health and quality of life in the community. Forward Solutions, located in Madison, WI, is one of the many programs provided by Journey Mental Health Center. Referrals come from several sources: Dane County Department of Human Services, Managed Care Organizations, Comprehensive Community Services, and Targeted Case Management programs. Forward Solutions provides comprehensive, ongoing services, following the principles developed by the Program of Assertive Community Treatment (PACT). Services include medication evaluation and monitoring, assistance in obtaining a source of income, locating housing, securing and maintaining competitive employment, counseling for alcohol and other drug use, and help with activities of daily living (e.g. grocery shopping, cleaning apartments, transportation, etc.) Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions. Requirements

  • Undergraduate, Masters, PsyD/PhD or Nursing
  • No experience or skills necessary.
  • Must have an openness to meeting new people, trying new things, a desire to help, and the willingness to reflect upon the system and the work
  • Must have valid drivers license, a favorable driving record, adequate insurance, and access to a vehicle for work (i.e. transporting consumers). Milage reimbursement at the Federal rate
  • Coursework with Social Work, Criminal Justice certificate, Psych Rehab, Community Mental Health, Nursing
  • Proficiency with Outlook, Zoom, and Teams.
  • Comfortable working in the community.
  • Comfortable working with individuals experiencing psychotic and affective symptoms.
  • Ability to develop clinical skillset during the internship.
  • Ability to develop an understanding of community resources, healthcare system, and criminal justice system.
  • Ability to establish and maintain boundaries
  • Commitment to work toward improving cultural sensitivity as demonstrated by valuing diversity, recognizing personal limitations in one’s competencies, and having the desire to improve.
  • Strong commitment to principles of recovery and to facilitating consumer participation in treatment/service planning and provision.
  • Strong verbal, writing, organizational, leadership and advocacy skills.

Responsibilities

  • The CSP Intern is responsible for assisting with clinical assessment, treatment planning, symptom management, and case management activities for several consumers in the program.
  • Provide high quality and high-fidelity services required by DHS 63-CSP Standards.
  • Develop and maintain skillset to provide services across the lifespan.
  • Provide services that are culturally sensitive, recovery oriented, trauma sensitive and honoring of the consumer’s perspectives and beliefs.
  • Coordinate with other community agencies/service providers as needed.
  • Arrange for the provision of crisis management services as clinically and programmatically appropriate and necessary
  • Provides a range of recovery orientated clinical services including outreach, assessment, individual and group therapy, advocacy, resource procurement, support to family and natural supports, assessment of substance abuse challenges, treatment planning, referral, and case management in a context that is supportive of the consumer’s culture, values and perspectives.
  • Completes accurate clinical records including intakes, assessments, treatment plans and updates, progress notes, discharge summaries, and other information in a timely manner.
  • Attend staff meetings, in-services and other program activities as requested
  • Participates in JMHC committees, trainings and activities as directed and/or approved
