Essential Job Functions: Develops and monitors employment outcomes related to the successful maintenance of employment for a caseload of employed individuals. Supports individuals to achieve career goals such as additional job responsibilities, increased pay, promotions, etc. Assists individuals with job maintenance by teaching personal and professional skills development that will facilitate successful employment and support employer’s objectives and needs. Facilitate communication between the individual, their employer and their support team. Maintains relationship with employer to ensure DPV Day Program’s Employment Services adequately meet the needs of their business. Assists and supports individuals to develop strategies and make informed choices in regards to self-advocacy, health and safety and career direction. Uses professional, positive and person first language and communication techniques to build rapport and relationships with service recipients, family members, the business community, internal and external agency personnel, etc. Maintains accurate records in regards to community based work assessments, progress reports, pertinent employer documentation and billing of services rendered. Remains current with and uses agency technology that enables the collection, compilation, evaluation and submission of data to appropriate sources as per established department timeframes. Monitor and adjust Units Utilized form to ensure appropriate utilization of units and proactively request critical revisions via team forms. Other Job Functions: Successfully completes all required Employment Services trainings that ensure the Job Coach delivers services consistent with best practices. Works as a cohesive team member with co-workers, internal departments within the agency, external organizations, etc. to support achievement of employment outcomes and assure excellent quality services. Adheres to all policies and procedures of the DPV Day Program Employment Services Department and DPD. This includes but is not limited to maintaining standards of confidentiality and best practices as well as performing and completing as needed duties and assignments in the best interests of services recipients, the Employment Services Department and DPV Day Program. Performs emergency duties as requested in the best interest of DPV Day Program and the health and welfare of its recipients and employees. Develop required reports and maintain individual files (i.e. monthlies, quarterlies, communication logs, incident reports). Upon authorization of a new individual, thoroughly review all important documents such as ISP and behavior support plan. Travel train individuals utilizing public transportation to successfully travel to and from work. Assist individuals to build relationships and natural supports in the workplace and advocate for themselves. Actively participate in all Pre-ISP and ISP meetings including employment updates. Attend all Employment Team Meetings and trainings. Complete daily case notes by noon the following day. Submit schedules to supervisor and timesheets by the end of each pay period. Complete monthly summaries. Submit employment placement and termination forms within 5 days. Check email and voicemail daily. Respond within 24 hours. Communicate with team members via email, phone calls and meetings. Visit with each individual at least once per month
