This Intern - Linkage to Care position involves participating in telemedicine outreach, assisting Health Navigators in connecting patients to follow-up care for communicable diseases, STIs, and SUDs, and documenting patient interactions, referrals, and outcomes in the electronic medical record (EMR) and related tracking tools. The intern will also collect, log, and maintain patient data with accuracy and confidentiality, handle protected health information (PHI) in strict accordance with HIPAA, and provide general administrative and operational support. Responsibilities include attending team meetings, case conferences, and cross-functional discussions, collaborating with the Practice Lead, Health Navigators, and clinical staff on active projects, and supporting special projects and pilot initiatives, including data collection, research, and process-improvement efforts. The intern is expected to attend training sessions and actively engage in the learning process.

Requirements

• Currently enrolled in a degree program at an accredited college or university in a field of study related to the internship, or a recent graduate (within the last 6-12 months).
• Work experience in an office or clinical environment, as applicable to the internship.
• Excellent verbal and written communication skills.
• Strong computer skills; proficient in Microsoft Office suite (Outlook, Word, Excel, PowerPoint).
• Ability to work independently and as part of a collaborative team.
• Organizational and customer service skills.
• Attention to detail.
• Demonstrated ability to handle detailed work accurately and quickly, work to deadlines on multiple tasks, and to work as a team member in a collaborative and participatory manner, and to interact both in person and remotely with a courteous and professional demeanor.

Responsibilities

• Participate in telemedicine outreach by placing outbound phone calls to patients and providers to facilitate linkage to medical, behavioral health, and social services.
• Assist Health Navigators in connecting patients to follow-up care for communicable diseases, STIs, and SUDs.
• Document patient interactions, referrals, and outcomes in the electronic medical record (EMR) and related tracking tools.
• Collect, log, and maintain patient data in Excel and Google Sheets with accuracy and confidentiality.
• Handle protected health information (PHI) in strict accordance with HIPAA and organizational privacy standards.
• Provide general administrative and operational support for day-to-day business transactions.
• Attend team meetings, case conferences, and cross-functional discussions; contribute observations and insights where appropriate.
• Collaborate with the Practice Lead, Health Navigators, and clinical staff on active projects, offering assistance and fresh perspectives.
