About The Position

Alexander Youth Network is seeking an Intensive In-Home Coordinator to assist in the day-to-day coordination of the Intensive In-Home program. This role involves providing brief behavioral and emotional services to children and their families within a community-based model. The coordinator works as part of a 3-person team, managing a maximum caseload of 12 families. Alexander Youth Network is dedicated to helping children overcome mental health challenges and offers a supportive, inclusive work environment with career development opportunities, mentorship, coaching, and pathways for advancement.

Requirements

  • B.A or B.S. or an Associates Degree in Social Work or a related human service field
  • Credentials of a Qualified Professional (QP)
  • Two years of documented experience with the child/adolescent mental health population
  • Proficiency in Microsoft Office applications
  • Valid Driver's License

Responsibilities

  • Assist in the day-to-day coordination of the Intensive In-Home program
  • Provide brief behavioral and emotional services to children and their families within a community-based model
  • Conduct skill-building activities
  • Provide parenting education
  • Intervene during a crisis
  • Provide on-call coverage within the intensive in-home team 24/7/365
  • Respond appropriately and timely to crisis situations
  • Develop, review, and follow individual Person Centered Plans with the Child and Family Teams for each child on their caseload
  • Provide psycho education to children and families
  • Facilitate effective step-down to less restrictive services and increased reliance on natural supports over time
  • Collaborate, coordinate and advocate with collateral agencies and natural supports in order to meet family and consumer needs in all domains
  • Provide services in the client’s home, school, and community settings, under the direction of the Intensive In-home Therapist
