About The Position

At Monarch, the Intensive In-Home Associate III is responsible for providing intensive family preservation services to improve individual and family strengths, stabilize the home environment, promote positive family interaction, and prevent the use of out-of-home placement. This role supports up to twelve individuals/families impacted by mental illness, substance use disorders, intellectual and developmental disabilities, and traumatic brain injury. Requirements

  • A Qualified Professional (10A NCAC 27G .0104) must have one of the following: Bachelor degree (non Human Services) with 4 years Full time OR 8 years Part time applicable experience required; Bachelor degree (Human Services) with 2 years Full time OR 4 years Part time applicable experience required; Master degree (Human Services) with 1 year Full time OR 2 years Part time applicable experience required.
  • Minimum of a Bachelor Degree (dependent upon experience).
  • Valid Driver's License (USA).
  • Qualified Professional (QP) - Monarch-DSM certification.
  • Experience working with children, adolescents, or transition-aged youth with a mental health diagnosis, an intellectual/developmental disability, or substance use disorder (1-4 years dependent upon education).

Responsibilities

  • Coordinate and oversee the initial and ongoing assessment activities, assist the Team Leader in identifying strengths and utilizing those strengths in order to help individuals and families achieve their stated goals/dreams.
  • Convene the Child and Family Team for person-centered planning.
  • Complete the initial development and ongoing revision of the Person-Centered Plan and ensuring its implementation.
  • Consult with identified medical (e.g primary care and psychiatric) and non-medical (e.g. DSS, school, DJJDP) providers, engage community and natural supports, and include their input in the person-centered planning process.
  • Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
  • Assist individuals and families in accessing community resources, including locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health, developmental disabilities and substance abuse services.
  • Provide and coordinate behavioral health services and other interventions for the youth or other family members with other licensed professionals and Child and Family Team members.
  • Monitor and document the status of the recipient's progress and the effectiveness of the strategies and interventions outlined in the Person-Centered Plan.
  • Participate in the person-centered planning process.
  • Assist with implementing a home-based behavioral support plan with the youth and his or her caregivers as indicated in the Person-Centered Plan.
  • Provide psychoeducation as indicated in the Person-Centered Plan.
  • Assist the team leader in monitoring and evaluating the effectiveness of interventions, as evidenced by symptom reduction and progress toward goals identified in the Person-Centered Plan.
  • Assist with crisis interventions.
  • Provide 24/7/365 on-call access to families served by the intensive in-home team and be a first responder to families in cases of emergencies.
  • Assist the team leader in consulting with identified providers, engaging community and natural supports, and including their input in the person-centered planning process.
  • Assist individuals and families in developing natural resources and make contact with social support networks.
  • Maintain necessary medical records in compliance with state, federal, and agency guidelines.
  • Attend educational trainings/seminars to remain up to date on current best practices and maintain required certifications and competencies.
  • Preserve positive working relationships within the communities served, including individuals, families, staff, monitoring and licensing agencies, organizations, funding resources, and other human service agencies.
  • Drive and travel extensively to community locations, various agencies, and other outreach destinations.
  • Provide and/or arrange for transportation for individuals and families as required.
  • Participate in the ongoing quality improvement activities of the agency.
  • Follow service definition guidelines for services being provided.
  • Demonstrate knowledge of and comply with all agency policies and procedures.
