About The Position

Intensive In-Home Therapy will be provided to children and families who are at risk for placement outside of their home/community or are at risk for hospitalization. The therapist is on-call to the child/family any time. The position requires the therapist to go to the home, school, or other place in the community to meet with the child/family, up to 10 hours per week. Other services will include developing a treatment plan, connecting the family with community resources, behavior interventions and teaching the family the interventions, developing a crisis plan, helping to form or modify an IEP if needed at school, providing intervention strategies for school personnel, and providing any needed service, intervention, or planning that might be needed. Job requires a Licensed Master Social Worker, Licensed Provisional Social Worker, Licensed Provisional Counselor, or Licensed Professional Counselor. Swing shift and varies M-F. Remote for administrative, in-office meetings twice monthly, client contact is conducted in-person and in the community. Provide services of counseling and case management to the family in their own environment for up to 10 hours per week. Develop a treatment plan. Develop a crisis plan. Use "wrap-around" philosophy to provide resources for the family to include respite services. Provide 24 hour/7 days a week on-call availability.

Requirements

  • Licensed Master Social Worker
  • Licensed Provisional Social Worker

  • Less than 1 year Applicable Experience
  • Provisional License - Various
  • Master's Degree
