About The Position

Responsible for completing new intake processing as well as following up with new referrals in a timely manner. Also, responsible for some transport responsibilities as assigned by the Admissions Manager.

Requirements

  • High School Diploma or equivalent (GED).
  • Must have knowledge of medical privacy and confidentiality (Health Insurance Portability and Accountability Act [HIPAA]); computer applications/software to include Microsoft Office programs/MS Outlook (e-mail), and internet familiarity.
  • Having a comfort level working with incarcerated individuals in a jail environment is a must.
  • Must have organizational and communication skills, both written and verbal.
  • Communicate effectively in oral and written format with i.e., attorneys, physicians, and other referring agencies.
  • Maintain effective interpersonal relationships.
  • Must have valid driver's license
  • Must meet insurance provider’s guidelines for insurability
  • Must have ability to work independently and work in a stressful and busy environment.
  • Office administration phones, copier, scanner, fax machines etc.
  • Running background checks with confidentiality
  • Moderate computer skills including but not limited to Microsoft Excel and Word and various web sites.

Nice To Haves

  • Experience in the behavioral health field a plus.
  • Electronic medical records experience is desired.
  • Experience working with individuals and families who have various mental, behavioral, and substance abuse conditions is a plus.

Responsibilities

  • Coordinate Referrals/Intakes
  • Transport of Clients and Shift Supervisor Duties
  • Works with Admissions Manager/Intake Team to contact all referrals in a timely manner.
  • Provides general information on services offered by Revive programs.
  • Gathers general information on potential client’s mental health needs, current level of crisis, payment options, legal status, and other relevant information.
  • Identifies factors which suggest the referral may be better served by another agency dependent on need and level of care.
  • If appropriate, provide contact information for other agencies.
  • Schedule applicant for admission date.
  • Oversee and manage orientation process for each intake.
  • Process Paperwork, Maintain Records in Timely Manner.
  • Provides needed documentation to enroll clients into residential treatment.
  • Coordinates and ensures timely processing of program documents including referrals, intakes, discharges
  • Determines client’s strengths and needs by establishing a safe trusting relationship with clients, families, and caregivers.
  • Data entry and completion of signing documents into electronic health record with intakes.
  • Establishes a compassionate and respectful environment by practicing good communication skills; keeping client information confidential; conducting self in professional manner.
  • Complete projects and tasks as assigned by supervisor.
  • Actively participates in daily/ weekly meetings and annual trainings.
  • Pick up clients at scheduled locations and times and transport to appropriate destinations in a punctual and timely manner.
  • Drive safely in accordance with traffic laws and posted speed limits while accounting for weather conditions.
  • Travel by routes specified by patient transportation manager or an approved GPS system
  • Set up and coordinate a walk-through on 30-minute timeframe to be performed every day around the clock 24/7 during your assigned shift.
  • Supervise and verify completion of client daily chores each day by set deadline.
  • Maintain an updated dorm list.
  • Administer drug screens, room checks, bodily searches or other searches as needed of all persons served.
  • Being familiar and well versed with and adhering to information provided in the Shift Supervisor Training Manual.
  • All other duties as assigned.
