Oversee and manage all hospice intake and referral processes for Care360 Hospice locations Answer incoming phone calls and receive all hospice referral and intake information in a timely, professional, and compassionate manner Complete referral and intake forms accurately and route documentation appropriately for admission approval for all Care360 Hospice locations Verify Medicare coverage and confirm benefit periods by reviewing common working files and payer systems Assist in scheduling appropriately skilled interdisciplinary team members to support new admissions and input scheduling information into the computer system Provide clear explanations of hospice services and benefits to patients, families, and community contacts Support marketing representatives with marketing and relationship-building activities to promote and maintain referral sources Supervise, train, and support Hospice Intake Coordinators to ensure accurate, timely, and compliant admissions Ensure intake documentation is complete, accurate, and routed appropriately for admission approval Monitor Medicare coverage verification, benefit period tracking, and payer compliance Serve as the escalation point for complex referrals, eligibility issues, or payer challenges Ensure compliance with CMS, Medicare, and hospice regulatory requirements Perform other duties as directed
