Intake nurse is a key position for building relationships in the community with our referral sources. Excellent customer service and communication skills are required. Requirements
- Current Pennsylvania LPN license
- Home health background preferred
- Strong interpersonal skills
- Positive, proactive attitude
Responsibilities
- Answers phones professionally
- Takes all necessary information on referrals to insure all demographics, diagnoses, and medical orders are complete
- Makes initial determination if patient can be accepted based on insurance type, patient location, and ability to staff
- Collaborates with clinical team to insure all accepted referrals are processed for admission and scheduling
