For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. Through the Benson Ridge and the Southwest Queens Senior Case Management Programs, case management and the homecare services are offered to the homebound elderly. The services include offering care management plans, assessments for the homecare, connections with the Home Delivered Meals and Friendly Visiting. The goal of the Case Management Program is to provide vital support services to keep frail seniors living safely in their homes and communities. STATEMENT OF THE JOB The Intake Coordinator provides a comprehensive review of Case Management services and screens potential older adult callers for eligibility for services. If deemed eligible, the intake worker inputs critical information to initiate the intake process and case enrollment in the NYC Aging database after obtaining the consent from the caller. The Intake Coordinator must also be able to communicate the case management services to other callers from the community.
