Requirements

  • Licensed Practical Nurse currently licensed to practice nursing in the state of North Carolina.
  • One-year of recent experience in an acute facility preferred.
  • Successful completion of skills assessment.
  • Possess and maintain current certification in CPR for cardiopulmonary resuscitation.
  • Must have good observational/nursing judgement and communication skills.
  • Licensed Practical Nurse currently licensed to practice nursing in the state of North Carolina

Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors.
  • Provides comfort to patient/family to facilitate quality of life and death with dignity; (a) performs competently all skills, treatments, and procedures as approved and allowed by the Practice Act of the N.C. State Board of Nursing; (b) Observes the patient's condition and reports changes of condition and the need for nursing beyond his/her knowledge to supervisor; (c) assist the patient with personal care, as well as basic nursing care; (d) initiates preventive nursing procedures, as appropriate for the patient's safety; (e) assist patient's family and other health care team members in providing continuity of care and meeting the emotional needs of the patient and immediate family members. Demonstrates proficiency in donning/doffing PPE and verbalizes knowledge of circumstances demanding PPE.
  • Maintains current electronic documentation and submits every shift; (a) ensures complete documentation on an ongoing basis supporting Hospice guidelines; (b) check for outstanding tasks daily and complete prior to leaving each shift, and (c) accurately use the time clock.
  • Teaches and counsels the patient/family regarding: (a) communicates problems of the patient(s) on a timely basis following chain of command; (b) communicates information using current process and technology available to 3HC; (c) coordinates hospice care with the Hospice Interdisciplinary team.
  • Maintains regular communication with supervisor; (a) communicates problems of the patient(s) on a timely basis following chain of command; (b) communicates information using current process and technology available to 3HC, (c) coordinates hospice care with the Hospice Interdisciplinary team.
  • Stays abreast of overall patient care and improves nursing care skills; (a) participates in monthly staff meetings at least 50% of time, hospice's quality assessment and performance improvement program, the interdisciplinary team in the development and implementation of the patient/family care plan, patient care conferences, record audits utilization reviews, quality improvement activities, and in-services; (b) attends and participates in interdisciplinary team conferences as directed; (c)attends agency mandatory in-services, hospice sponsored in-service training/continuing education seminars as required and satisfies CEU requirements of the State Board of Nursing; (d) strives to improve nursing care through continuing education, active participation in professional and related organizations and individual research and readings, and (e) takes responsibility for own actions and for seeking sufficient information necessary to function effectively at the impatient facility.
  • Works in GIP level of care when necessary, accompanied by a RN, and administers required meds. (PO, SQ, IV) prn, enters skilled need note in EMR on assigned patients in GIP. Enters timely IDG notes for Residential patients.
  • Demonstrates an interest in personal and professional growth: (a) attends and participates in workshops, mandatory in-services, seminars, and webcasts to keep abreast of current changes in rules, regulations relating to job functions; and (b) does individual reading and research.  
  • Demonstrates a willingness to be cost-effective in the use of Agency resources, the monitoring of waste, and the proper and safe use of supplies and equipment.
  • Adheres to 3HC's Personnel Policy and performs other performs other duties as assigned by supervisor to promote the successful operations and future growth of 3HC.
