About The Position

Our client is seeking an Independent Living Specialist to assist disabled New Yorkers as they transition from institutional settings or unstable living situations into their own apartments. The specialists will work with program participants to connect them with support services, serve as a liaison to community service providers and otherwise support their transition. The specialists also will work with colleagues at other independent living centers and the state-wide program.

Requirements

  • Significant experience in helping people transition to independent living situations, including expertise in the development of Individual Service Plans.
  • Experience with the concepts of the Independent Living movement and participant-directed control of their lives.
  • A strong commitment to promoting and advocating for the full integration of disabled people in the community.
  • Experience helping people find affordable and accessible housing. Knowledge of housing policies, laws and issues that affect people with disabilities and seniors.
  • Familiarity with the state’s Medicaid program and other programs intended to get people living independently in the community.
  • Ability to work independently and cooperatively with others.
  • Strong verbal, written, computer and organizational skills essential.
  • Bachelor’s degree or equivalent experience
  • Personal experience with disability preferred.

Nice To Haves

  • Spanish, ASL or other language skills a plus.

Responsibilities

  • Develop Individual Service Plans (ISPs) with program participants, centering their needs and goals. The ISP will include plans to connect with community resources, including budgeting, transportation and health care resources.
  • Establish connections with nursing homes, hospitals, service providers, managed care organizations, Performing Provider Systems, and other providers.
  • Advocate with applicants with their housing specialists; transition specialists, nursing home discharge planners; care managers; family, friends and neighbors.
  • Advocate with the participant to ensure supports are in place for admission to the program and transition to independent living.
  • Track and report on all activities using a statewide project database.
  • Meet with participants in their homes to assess whether the ISP is working and to make adjustments if necessary.
  • Other duties as required, including community outreach events.
