About The Position

For more than 126 years, Epworth Children's Home has grown and adapted to the individual needs of children and families by providing a safe and nurturing environment, evidence-based practices, and programs across the continuum of care and across the state of South Carolina. Position Summary: The Independent Living (IL) Case Manager Document case management services and the progress of the client through maintaining up-to-date files on each client in the program, completing intake assessments, monthly and discharge reports on each client, care and transition plans, life skills and other assessments, session notes, life skills notes, incident reports, observation notes, and completing all other assigned paperwork

Requirements

  • Master's degree in a Human Service field or a Bachelor's degree and one (1) year experience in residential group care or an Associate's Degree and two (2) years experience in residential child group care or four (4) years of experience at Epworth
  • Has a belief system and engages in faith practices that are compatible with those of the United Methodist Church

Nice To Haves

  • Prefer a minimum of four (4) years of experience working with children and families, three (3) of which should be in residential group care
  • Prefer a minimum of one (1) year in a counseling or social worker capacity

Responsibilities

  • Understands and follows personnel policies and practices as outlined in the ECH Staff Manual and in compliance with the SCDSS group home regulations as well as those outlined in the Fixed Price Bid Contract with the South Carolina Department of Social Services, and the C.A.R.E. practice model
  • Attend individual and group supervision meetings and training as scheduled
  • Attend all Interdisciplinary Meetings
  • Respond to behavior crises in the center whenever needed
  • Complete intake and follow up assessments on all clients, document assessment reviews quarterly
  • Schedule physicals, dental appointments and vision appointments for clients
  • Ensure clients reach self-sufficiency goals and document progress on care plans.
  • Assist clients with finding housing and employment
  • Ensure clients have access to programs for academic, employment, health care, mental health support, and other necessary resources
  • Accompany clients to medical and clinical appointments, psychiatric appointments, and hospitalizations and follow all documentation procedures related to this process
  • Function as a member of an interdisciplinary team cooperating with and involving others to obtain common goals for each client
  • Teach life skills through group and individual lessons.
  • Hold Review Meetings on each client involving the placing agency (if applicable), client, service providers, and all interdisciplinary team members
