About The Position

Lead and conduct thorough root cause investigations to ensure accuracy, completeness, and compliance with organizational standards. Serve as the Investigation Team Leader, guiding cross-functional teams in identifying underlying causes and corrective actions. Utilize investigation databases such as KMS to document findings and maintain detailed, accurate records. Verify proper classification of incident types and ensure timely completion and finalization of investigation reports. Apply strong analytical and problem-solving skills to assess evidence, conduct effective investigation interviews, and drive continuous improvement through actionable insights.

Requirements

  • A minimum of six (6) to ten (10) years of applicable process chemistry or incident investigation experience within a petrochemical or refining site, depending on the level of education.
  • Knowledge of EHS and PSM procedures.
  • Excellent written and oral communication skills are required.
  • Experience reading and interpreting P&IDs, federal law requirements, and procedures.
  • Strong computer skills, including Microsoft Office and KMS, or similar experience.
  • Trained in or certified in Cause Mapping or other similar root cause analysis methodology.
  • Proficiency in data analysis and visualization tools (e.g., Excel, Power BI) to identify trends and support continuous improvement in incident investigations.
  • Must have regular, reliable attendance as well as the ability to work outside the regular schedule as required, including frequent overtime for time-sensitive projects
  • Must demonstrate exceptional teamwork and customer service; behave professionally; treat others with respect and dignity
  • Must be able to handle and manage the stress associated with the position, often changing from one task to another of a different nature.
  • High School diploma or equivalent required with (10) years of above-mentioned experience.
  • Bachelor's degree [preferred] in engineering disciplines, safety, or PSM-related disciplines with 6 years of experience or an Associate's Degree with 8 years of above-mentioned experience.

Nice To Haves

  • Preferred experience with leading incident investigations utilizing Cause Mapping or root cause analysis methodology.

Responsibilities

  • Lead and Facilitate Investigations: Serve as a problem solver, facilitating and guiding incident investigations at the site level as requested. Ensure investigations are conducted effectively and efficiently, applying root cause analysis and consistent methodology. Present finalized investigation findings and recommendations to management as required.
  • Quality Assurance: Perform quality assurance reviews on investigations submitted by site team leaders to ensure accuracy, completeness, and adherence to established methodologies and formatting standards. Provide feedback and guidance to promote continuous improvement in investigation quality.
  • KMS Reviewer: Act as a designated reviewer for TX incident records initiated and closed in KMS, ensuring documentation accuracy and compliance with internal standards.
  • Support: Provide guidance and coaching to site-level team leaders to strengthen investigation competency and ensure detailed, thorough reports. Offer day-to-day support to management on all aspects of incident investigations. Support investigation-related training activities and assist in procedure revisions, including the development and application of tools and resources. Collaborate with the KMS team on incident trending and analysis, as needed. Coordinate with third-party consultants to achieve investigation improvement goals. Assist in planning and facilitating quarterly TX site incident investigation networking meetings.
  • Training and Development: Conduct one-on-one and small group training sessions with site facilitators to enhance knowledge, skills, and consistency in investigation practices. Foster a culture of continuous learning and improvement to elevate the overall quality of site investigations.
