About The Position

The In-Home Services Scheduler is responsible for the coordination of in-home services for all InnovAge PACE participants. The In-Home Services Scheduler prepares schedules according to participant needs, staff requests and within the plan of care designed by the interdisciplinary team. Schedules staff in a way that meets the needs of participants. Coordinates and communicates schedule and issues with the transportation department to ensure a smooth-running operation. Notifies participants and families regarding changes in their schedules, tries to notify as soon as information becomes available so as not to inconvenience families and participants. Reports pertinent participant events or missed visits to appropriate Home Care Coordinator. Prepares and maintains daily, weekly, and/or monthly service schedules, logs, ledgers, tickler systems and other records in electronic or written form. Communicates with outside agencies and independent contractors regarding services rendered on behalf of IGCP. Attends meetings, as required by Supervisor. Assists with orientation and training of IHS field staff and attends IHS departmental meetings. Assists with filing and other clerical office functions as requested.

Requirements

  • 2 years training in health care delivery, in home health
  • 1+ year scheduling and/or dispatching
  • High School Diploma or GED
