About The Position

This is a Disease Intervention Specialist providing Linkage to Care services. This position requires firsthand knowledge of public health systems, as well as professional and interpersonal skills to interact with health care clients, health care professionals and others who impact STD Prevention and Control, public health awareness and other CHD activities. Duties will also require knowledge of varied social, economic, cultural and lifestyle characteristics, ability to work in a fast-paced environment and a skill in communicating with and eliciting information from patients and target groups with these diverse backgrounds. Minimal guidance and instruction on work methods, procedures and techniques are provided with a review of activities at completion for technical accuracy, timeliness and adherence to instructions and guidelines. The incumbent is required to link clients to medical treatment services and must be able to work collaboratively with clinical and other key staff that plays a role in providing Linkage to Care services to the clients of Miami Dade County. All activities performed must comply with federal, state, department and CHD information Security and Confidentiality statutes, rules, regulations, policies and procedures. On-medical case management duties including but not limited to: Initial comprehensive assessment of service needs.  Timely and coordinated access to medically appropriate levels of health, prevention and support services and continuity of care.  Eligibility Determination for Ryan White Part B services.

Requirements

  • Knowledge of methods of compiling, organizing and analyzing data.
  • Customer and Personal Service - Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
  • Communicating with Supervisors, Peers, or Subordinates — Providing information to supervisors, co-workers, and subordinates by telephone, in written form, e-mail, or in person.
  • Ability to maintain strict confidentiality.
  • Judgement and Decision Making – Considering the relative costs and benefits of potential actions to choose the most appropriate one.
  • Valid driver’s license, access to reliable vehicle to perform job related duties, experience working in a disease control program, social services program, or community-based organization.

  • Incumbent in this position is preferred to be bilingual in English and Spanish.

Responsibilities

  • Locates and links clients diagnosed with HIV to medical treatment services as well as any other services needed.
  • Performs case management activities to include preparing and maintaining timely and accurate records and reports of the linkage to care process for program evaluation purposes and other records and reports as required and submits to the Monitoring and Evaluation Coordinator.
  • Provides PCRS in accordance with current STD HIV/AIDS Program policy to a wide variety of individuals infected with or at-risk of STD, hepatitis, HIV/AIDS or other bacterial, biological and/or viral agent to elicit partner and suspect information.
  • Assistance in additional office-related activities and duties as assigned.
  • Perform other related duties as assigned.
