HUMAN SERVICES COUNSELOR I Your Specific Responsibilities: Establish a trusting relationship with culturally diverse families by initiating and maintaining regular client contact. Provide Case Management to pregnant women and infants referred to the Healthy Start Program. Provide face to face contact with clients either in the OB provider’s office or in the client’s residence. Provide Parenting Education one on one with clients. Provide Breastfeeding Education and Tobacco Cessation Education to individual clients. Document all client contact information in the web-based computer application known as the Well Family System Provide referrals to community resources to supplement Healthy Start services. Required Knowledge, Skills, and Abilities: Experience in Counseling techniques and strategies Experience working with pregnant women and children. Problem solving skills related to advancing client self-sufficiency. Ability to work well with others as a team. Computer skills: competency and experience. Experience in interviewing and assessing to determine the needs of the client. Knowledge of community resources. Qualifications: Minimum- Valid Florida Driver's License At least 1 year experience working with high-risk pregnant women and/or families Must have reliable and dependable transportation every day for home visitation purposes Preferred- 1 to 3 years of experience working with families 1 to 3 years home visiting experience 1 to 3 years case management experience Bilingual with fluency in English and Spanish Associate's Degree or Higher in psychology, Social Work or related field
