Job Details Our Hub Manager (RN) will: Collaborate and coordinate with multidisciplinary teams to facilitate a safe quality experience for patients through the continuum of care. Evaluate bed requests and assign beds on a priority basis to facilitate flow. Efficiently place each patient and verifies he/she is in the appropriate level of care and correct patient status/type. Serve as an expert resource in patient flow, status and progression and provides ongoing education to staff, physicians, patients, and their significant others. Provide centralized one-call service for direct admits and facility transfers. Evaluate appropriateness of admission (patient status, level of care) for direct admits and facility transfers. Liaison between all patient care services to ensure appropriate bed is assigned, patient care needs are met and treatment plans are executed in the appropriate level of the continuum. Recognize and take appropriate action in situations that require immediate intervention. Manage Hub/facility staffing for assigned and next shift, if applicable. Collaborate with Nursing Supervision, Nursing Leadership, MCC’s and Order Organizer to ensure patient progression through appropriate allocation of resources. Proactively identify and remove barriers that may impede department or system-wide patient flow. Meet deadlines and exceed metrics identified for the Facility/ Unit through attention to detail and implementation of facility policies and Standard Operating Procedures (SOPs)

Requirements

  • Passion for Care Management
  • 3+ Years acute care clinical RN experience required
  • 2+ Years leadership experience preferred
  • Prior RN experience in Utilization Management, Performance Improvement, Transition Planning and/or Care Management experience in an acute hospital or related managed care setting preferred
  • Education: Graduate of an accredited School of Nursing required
  • Bachelors of Science in Nursing (BSN) degree required
  • Applicants currently enrolled in a BSN program may apply only if they are currently in a Care Manager role
  • Licensure: New York Registered Nurse (RN) License & Registration
  • Certification: Basic Life Support (BLS) certification required; PRI certification required
