New Freedom’s Care Coordination Program Manager plays a crucial role in delivering exceptional care coordination services to our members within our intensive outpatient and supportive housing facilities. In this position, the Care Coordination Program Manager assumes responsibility for coordinating all clinic, wellness, and laboratory operations onsite, aligning them with regulatory and compliance standards as well as with our company mission and values. Additionally, they actively collaborate with services to ensure our members have access to the best resources available. Beyond these responsibilities, the Care Coordination Program Manager serves as the manager of our internal Care Coordination department. They provide guidance and support, going above and beyond to surpass the continuum of care needs of our members.
