About The Position

The Manager, Care Coordination is responsible for the leadership, development, and oversight of care coordination services in outpatient and community-based settings with a team comprised of Health Navigators, RNs, and Medical Social Workers. This role focuses on advancing population health outcomes through proactive outreach, care management, and interdisciplinary collaboration. The Manager ensures high-quality, patient-centered care while optimizing resource utilization, improving access, and reducing health disparities across defined patient populations.

Requirements

  • Current California Registered Nursing (RN) License required.
  • Bachelor’s degree in nursing, healthcare administration, public health, or a related field required.
  • Current Basic Life Support (BLS) certification required
  • Minimum of six (6) years registered nursing experience required
  • Minimum of two (2) years of supervisory experience required
  • Knowledge of HIPAA, the Joint Commission, CMS, and other applicable regulatory standards/regulations required
  • Knowledge of community resources and healthcare financial systems required
  • Experiences where leadership, integrity, initiative, clinical skills and effective communication skills have been demonstrated required
  • Exceptional verbal and written communication skills, with the ability to prioritize and delegate work effectively.
  • Demonstrated team building, group facilitation skills, expertise in change management, and the ability to communicate respectfully with all team members.
  • Proficiency in using data analysis tools, including but not limited to Microsoft Excel.
  • Must exhibit flexibility, adaptability and willingness to change or try new approaches.

Nice To Haves

  • Certified Case Manager (CCM) certification preferred.
  • Previous Case Management, Population Health, Care Coordination, outpatient clinic and/or Home Health experience preferred
  • Experience in non-traditional care settings such as “street nursing” preferred
  • Understanding of healthcare system operations, patient navigation, and health plan structures, including self-funded plans and Accountable Care Organizations / value-based care models strongly preferred.
  • Knowledge or experience in quality improvement methodologies, health intervention strategies, and project management is strongly preferred.
  • Familiarity with Epic EMR is preferred.
