The Housing Case Manager role is a case management role that works directly with program participants to develop, arrange, and coordinate housing focused case management services through the CalAIM (California Advancing and Innovating Medi-Cal) Community Supports Program. Participants are adults and children with families. The primary focus is to provide comprehensive Housing Navigation and Housing Tenancy case management services to members who have chronic health and/or mental health conditions who may be experiencing homelessness or at risk of homelessness. The Housing Case Manager would be responsible for all case management activities associated with the designated program.
