About The Position

The Housing Case Manager provides therapeutic case management services for Coastal Horizons housing, homeless, and residential levels of care. This role involves screening, admission, and orientation for these programs, as well as assessing clients' life domain areas. Key duties include linking clients to community services, assisting with income and employment, coordinating service linkages, and managing medical and medication needs in conjunction with client medical requirements. The case manager will also conduct home visits/inspections, gather, enter, and document client information and interactions, and participate in local Continuum of Care meetings to enhance communication and continuity of care. Additionally, the position requires providing or arranging transportation for clients to various appointments and monitoring their compliance with program expectations.

Requirements

  • Bachelor's Degree within human services
  • Experience and skills in substance use and/or mental health case management
  • Trained to provide evidence-based care coordination, clinical screens, and to support treatment recommendation follow through
  • Possess the interpersonal skills required to comfortably approach and engage with the client population in diverse settings
  • Ability to assertively interact with and monitor client behavior
  • Strong interpersonal communication skills; ability to develop and maintain professional boundaries and to interact effectively with clients and staff
  • Knowledge of available community resources and services to support clients with substance abuse problems
  • Ability to represent the Agency in community forums and effectively conduct advocacy efforts aimed at furthering the vision and mission of Coastal Horizons Center
  • Knowledge of the laws, regulations and policies governing mental health and substance abuse treatment, residential services and supportive housing
  • Ability to demonstrate effective oral and written communication
  • Strong organization skills; ability to develop, organize and maintain schedules
  • Knowledge and comfort in working with client population
  • Basic computer skills
  • Ability to network and coordinate efforts between treatment, medical, and other support systems for the clients
  • Must have a valid NCDL and a good driving record approved by CHC insurance provider

Responsibilities

  • Provides therapeutic case management services for Coastal Horizons housing, homeless and residential levels of care
  • Screens clients for these programs
  • Orients clients for these programs
  • Coordinates admissions to the HHH Program
  • Participates in admissions and discharges for the program
  • Responds to emergency client needs such as medication and food when necessary
  • Assesses client’s life domain areas
  • Links clients to services in the community
  • Assists with income assistance and employment
  • Coordinates service linkage
  • Coordinates medical and medication needs in coordination with client medical needs
  • Assists clients in accessing pharmaceutical care charity programs, or charity care when applicable
  • Works with medical staff to coordinate services for the client
  • Conducts home visits/inspections
  • Gathers, enters, and documents client information and interactions
  • Participates in local Continuum of Care meetings in order to enhance communication and the continuity of care
  • Develops and maintains a network of supportive agencies in order to ensure necessary referrals for basic needs and other services that could include employment or school
  • Communicates and collaborates with apartment owners and landlords, to secure safe housing, to address any conflicts or problems, and to maintain positive relationships with housing agencies
  • Engages in regular consultation with team members and supervisor to ensure ethical and high-quality services and documentation
  • Coordinates with QRT, ORHP and Homeless Outreach Teams at Coastal Horizons
  • Attends weekly team meetings
  • Conducts monthly case management sessions with participants to further goals of permanent supportive housing
  • Transports clients utilizing a company car or van to and from treatment, medical appointments, and other community resource appointments
  • Monitors client compliance with program expectations
