About The Position

The Hospital Liaison is a key member of the OhioRISE team, serving as a bridge between hospitals, care coordinators, and community resources to ensure youth receive timely, coordinated, and supportive transitions of care. The Hospital Liaison will work directly with hospital partners to support discharge planning, provide immediate linkage to resources after hospitalization, and develop initial safety plans for new OhioRISE members. This role requires strong relationship‑building skills, clinical judgment, and the ability to collaborate across systems.

Requirements

  • LSW/LPC required
  • Strong clinical assessment skills and familiarity with behavioral health crisis response.
  • Exceptional relationship-building and interpersonal communication abilities.
  • Demonstrated initiative, creativity, and ability to develop new processes or programs.
  • Experience collaborating with multidisciplinary teams.
  • Strong organizational skills and ability to manage multiple cases and priorities.
  • Experience working with hospitals, emergency departments, or crisis stabilization programs.
  • Knowledge of community mental health, Medicaid services, and youth behavioral health.
  • Familiarity with OhioRISE or care coordination models.
  • Required CPR certification within 90 days of hire.
  • Required CPI certification within 90 days of hire.
  • Must be at least 21 years of age.

Responsibilities

  • Act as the primary liaison between hospitals and OhioRISE care coordination teams.
  • Collaborate with hospital staff to support effective discharge planning and transitions of care.
  • Connect youth and families to services and supports immediately following hospitalization.
  • Develop initial safety plans in partnership with families and treatment teams.
  • Monitor inpatient stays, continuity of care, and readmission trends.
  • Facilitate communication among hospitals, physicians, care coordinators, and community providers.
  • Attend hospital treatment team meetings and provide care coordination updates.
  • Maintain accurate documentation in compliance with agency and regulatory standards.
  • Identify barriers to care and support process improvements in hospital workflows.
  • Participate in team meetings, supervision, and ongoing training as required.
