About the Role CareM is seeking an experienced Hospice Quality Assurance Coordinator to support excellence across our hospice division in San Antonio. This role is ideal for a nurse who finds purpose in accuracy, clarity, and coaching — someone who brings steadiness to complex situations and elevates the work of others. In this role, you’ll ensure documentation quality, strengthen regulatory compliance, and partner closely with clinicians and leadership to support outstanding patient care.

Requirements

  • Registered Nurse (RN) or Licensed Vocational Nurse (LVN) with current Texas licensure
  • Prior hospice QA experience required
  • Strong understanding of hospice documentation, regulatory requirements, and Medicare guidelines
  • Exceptional attention to detail and follow-through
  • Strong communication skills with the ability to teach, clarify, and coach
  • Collaborative mindset with comfort working across teams, disciplines, and locations
  • Ability to navigate multiple priorities while maintaining calm, accuracy, and consistency

Responsibilities

  • Review clinical documentation for accuracy, completeness, and alignment with regulatory requirements
  • Conduct Plan of Care audits, chart reviews, and identify documentation trends
  • Serve as liaison between QA, field clinicians, and clinical leadership to resolve discrepancies and clarify expectations
  • Maintain deep knowledge of Medicare Conditions of Participation and hospice regulatory standards
  • Provide coaching and education to clinicians to strengthen documentation quality
  • Support survey readiness and participate in QAPI initiatives and quality improvement projects
  • Collaborate with leadership on process improvements and documentation best practices
