About The Position

A Hospice Intake Specialist collects, verifies, and enters patient information to ensure timely and accurate initiation of hospice services.

Requirements

  • Proficiency in verbal, written, and computer skills
  • Excellent communication, organization, and follow-up abilities
  • Ability to manage multiple assignments simultaneously
  • Strong attention to detail and the ability to work independently
  • Minimum of one (1) year of medical office experience required
  • Knowledge of home health and/or hospice operations preferred
  • Regularly required to walk, sit, stand, bend, reach, lift, and move about
  • Ability to communicate effectively, both orally and in writing

Responsibilities

  • Verify patient eligibility and benefits via phone and online portals for Medicare and various insurance plans
  • Complete intake for new patient admissions in the EHR system and schedule initial care visits
  • Obtain required authorizations for services prior to admission
  • Contact patients prior to admission to obtain and verify information and notify them of any financial responsibility, when applicable
  • Communicate effectively and in a timely manner with agency staff
  • Establish and maintain positive working relationships with agency staff and coworkers
  • Support organizational culture by promoting a Friendly, Positive, and Excellence-Focused environment
  • Perform other duties as assigned
