Hospice Intake, LPN
About The Position
Good Samaritan Hospice is looking to hire an Intake LPN Coordinator to support our team in accepting patient referrals from hospitals, health care facilities, and physicians for hospice care. The Intake LPN assists the caller in understanding the hospice services that can be provided through the agency. This is a full time position working Monday through Friday with a weekend rotation (every third), and is based out of the Valencia Office. Requirements
- Licensed practical nurse with a current valid Pennsylvania license
- Minimum of two [2] years of current experience as a registered nurse or licensed practical nurse in health care
- Knowledge base of agency policies, federal and state regulations and third-party payers
- Skilled in critical thinking, communication, negotiation and collaboration
Nice To Haves
- one [1] year of home health experience preferred
