We’re growing and looking for an experienced, compassionate Intake Coordinator to join our high-performing team! As an Intake Coordinator, you’ll be the first point of contact for patients and families, ensuring a seamless intake experience. You’ll manage referrals, verify insurance, coordinate care, and maintain strong relationships with referral sources—all while upholding compliance and quality standards.

Requirements

  • 2+ Years of healthcare referral experience required; home health or hospice intake preferred
  • Strong communication, negotiation, and public relations skills
  • Demonstrates autonomy, flexibility, and teamwork

Responsibilities

  • Oversee daily patient intake and referral operations
  • Ensure compliance with state, federal, and Joint Commission regulations
  • Implement efficient intake procedures and workflows
  • Build and maintain relationships with referral sources
  • Coordinate patient education and transition to home care
  • Manage insurance verification and authorization processes
  • Negotiate service pricing with payers within financial guidelines
  • Support budget planning and resource allocation for the intake department
  • Monitor and report referral trends and customer feedback
  • Maintain knowledge of community resources and contractual obligations
