We’re growing and looking for an experienced, compassionate Intake Coordinator to join our high-performing team! As an Intake Coordinator, you’ll be the first point of contact for patients and families, ensuring a seamless intake experience. You’ll manage referrals, verify insurance, coordinate care, and maintain strong relationships with referral sources—all while upholding compliance and quality standards.
Requirements
- 2+ Years of healthcare referral experience required; home health or hospice intake preferred
- Strong communication, negotiation, and public relations skills
- Demonstrates autonomy, flexibility, and teamwork
Responsibilities
- Oversee daily patient intake and referral operations
- Ensure compliance with state, federal, and Joint Commission regulations
- Implement efficient intake procedures and workflows
- Build and maintain relationships with referral sources
- Coordinate patient education and transition to home care
- Manage insurance verification and authorization processes
- Negotiate service pricing with payers within financial guidelines
- Support budget planning and resource allocation for the intake department
- Monitor and report referral trends and customer feedback
- Maintain knowledge of community resources and contractual obligations
