Inform referral sources, clients, family members, and physicians about response time and plans for follow up. Informs clinical office staff of potential admits and accurately documents information in the clients medical record. Input patient information into the EMR and updates EMR with the correct level of care. Completes and copies necessary paperwork to ensure patient files are prepared for admission. This includes but is not limited to completing the referral intake form, copying all referral information, and attaching it to the intake for review by clinical office staff, scanning and uploading referral information in a timely manner so information is available to admissions clinicians, Establishes, maintains, and closes patients EMR and paper charts in accordance with regulatory and office guidelines. This may include but is not limited to the mailing, faxing, or emailing of required documentation. Verification of insurance coverage. Collaborates as needed to procure needed information. Requirements

  • Associates degree is preferred and equivalent with successfully completed course work or equivalent experience in medical terminology and/or working in healthcare environments.
  • Minimum of two years experience working in hospice, home care, or other health care environment required.
  • Customer service focused, including the ability to set the caller at ease and encourage open dialogue about the clients needs.
  • Ability to work with a variety of team members representing other functional areas that directly or indirectly impact client intake/admission.
  • Ability to maintain composure in addressing stressful and/or challenging situations.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

Nice To Haves

  • Experience in dealing with a variety of payers in healthcare, e.g., Medicare, Medicaid, and commercial payors, preferred.
  • Prior use of Electronic Medical Records (EMR), preferred.
