About The Position
Mission Hospice & Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end-of-life support and education. Founded in 1979, we are a local, independent nonprofit that has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life-threatening diagnosis through death and bereavement. We are seeking an LVN who will be responsible for providing direct patient care to hospice patients at our Hospice House in Redwood City. Schedule: Per diem; must be available at least 4 shifts per month Location: Redwood City Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.
Requirements
- Completion of a Licensed Vocational Nursing Education Program from an accredited school.
- Minimum of one year of LVN experience preferably in an acute care setting.
- Current Licensed Vocational Nurse certification from the California Board of Vocational Nurse and Psychiatric Technician Examiners.
- Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.
Nice To Haves
- Experience in a home health/ hospice setting preferred but not required
- Valid California certification in Intravenous Therapy & Blood Withdrawal preferred
Responsibilities
- Provides direct patient care under current nursing practice following a comprehensive assessment and the plan of care.
- Provide accurate and timely documentation of patient care services.
- Assess and provide patient and family/caregiver education and information pertinent to plan of care.
- Participates in the coordination of hospice services and report identified needs to the Program Director and interdisciplinary team.
