About The Position

The Homeless Alliance works to rally our community to end homelessness by bringing together organizations to collaborate, identifying and filling gaps in services, streamlining processes, and using research-based strategies and national best practices to get people off the streets. The mid-sized organization has served Oklahoma City for 20 years and is considered a subject matter expert on homelessness. The Case Manager for the Hope PSH Program is responsible for providing housing-focused case management to individuals experiencing chronic homelessness who are enrolled in a Permanent Supportive Housing (PSH) program. This role supports up to 40 participants and works in partnership with Hope Community Services, who provide clinical and behavioral health support. The Case Manager’s primary responsibility is to assist participants with obtaining and maintaining stable housing and connecting to mainstream resources. This position uses a Housing First, trauma-informed, and person-centered approach.

Requirements

  • Commitment to the mission of ending homelessness with a Housing First and equity-driven approach.
  • Strong organizational skills with the ability to manage a large caseload and competing priorities.
  • Experience working with individuals with complex needs including behavioral health, trauma, and substance use.
  • Knowledge of local housing resources, landlord/tenant laws, and community supports.
  • Ability to work independently in the field and as part of an interdisciplinary team.
  • Strong written and verbal communication skills.
  • High tolerance for ambiguity and ability to adapt in a fast-paced environment.
  • Bachelor’s degree in social work, human services, or a related field preferred.
  • Two or more years of case management experience, ideally in housing or homelessness services.
  • Valid Oklahoma Driver’s License and reliable transportation with proof of insurance required.

Nice To Haves

  • Lived experience of homelessness, behavioral health, or justice involvement welcomed and valued.
  • ODMHSAS Case Management Certification and SOAR training preferred but not required.

Responsibilities

  • Manage a caseload of up to 40 participants in PSH who have experienced chronic homelessness and have disabling conditions.
  • Support participants with all housing-related tasks including housing navigation, completing lease agreements, coordinating move-in logistics, and addressing tenancy issues.
  • Assist participants in obtaining vital documents such as ID, Social Security cards, and proof of income.
  • Collaborate closely with Hope Community Services staff to ensure a coordinated and holistic service plan is in place.
  • Use Motivational Interviewing, harm reduction, and Housing First practices to engage participants and build trusting relationships.
  • Conduct home visits and provide ongoing support to ensure housing stability, including budgeting, lease compliance education, and landlord communication.
  • Link participants to employment, benefits, transportation, and other community-based services that promote long-term stability.
  • Maintain timely and accurate documentation of all participant interactions and progress in the Homeless Management Information System (HMIS) and agency records.
  • Participate in team meetings, case conferencing, and coordinated entry efforts.
  • Provide crisis intervention and support as needed to reduce risk of eviction or harm.
  • Transport participants to appointments and housing-related errands as needed.
  • Other duties as assigned.
