The Case Manager for the Hope PSH Program is responsible for providing housing-focused case management to individuals experiencing chronic homelessness who are enrolled in a Permanent Supportive Housing (PSH) program. This role supports up to 40 participants and works in partnership with Hope Community Services, who provide clinical and behavioral health support. The Case Manager’s primary responsibility is to assist participants with obtaining and maintaining stable housing and connecting to mainstream resources. This position uses a Housing First, trauma-informed, and person-centered approach.
